Domestic, family and sexual violence is a profound health issue. Its impacts on physical and mental wellbeing are long-lasting and far-reaching, affecting not only individuals but also families, communities, and the health system as a whole.

The Hunter New England and Central Coast Primary Health Network’s Primary Care Outreach Program trials new and innovative solutions to reach women and children in places where they feel safe, supported and able to access care.

The Toolkit captures what has been have learned so far and provides practical guidance for others who want to replicate or adapt this model of care. It is a resource built from evidence, co-design, and the lived experiences of women and children.
